Position Summary
The MDS Coordinator is responsible for oversight and coordination of the Resident Assessment Instrument (RAI) process, ensuring accurate and timely completion of MDS assessments, care planning, and regulatory submissions in accordance with federal and state requirements.
This position plays a critical leadership role in supporting appropriate reimbursement, quality outcomes, and interdisciplinary collaboration, while driving compliance, education, and continuous improvement. This role reflects the facility’s commitment to truthful documentation, excellence in care planning, and outcome-oriented clinical practice.
Key Responsibilities
RAI Process & Clinical Documentation (Primary Role): Oversee and manage the RAI process, including MDS assessments, CAAs, and interdisciplinary care plans
Ensure accurate and timely completion and submission of all MDS assessments in compliance with CMS and state regulations
Audit clinical documentation to support accurate coding and reimbursement (Outcome Oriented)
Maintain ARD scheduling and ensure adherence to regulatory timelines
Ensure care plans accurately reflect resident needs and services provided
Reimbursement & Regulatory Compliance: Support appropriate Medicare and Medicaid reimbursement through accurate PDPM coding
Collaborate with billing, therapy, and finance teams to ensure alignment and accuracy
Stay current on CMS regulations, PDPM updates, Quality Measures (QMs), and survey readiness expectations
Identify and address documentation gaps that may impact compliance or reimbursement
QAPI & Data Oversight: Track, trend, and analyze case mix index (CMI), Quality Measures, and resident acuity
Participate in and support QAPI initiatives through data-driven insights and reporting
Identify trends and implement process improvements to enhance outcomes and compliance (Excellence & Outcome Oriented)
Collaboration & Education: Lead and participate in interdisciplinary care plan meetings
Educate nursing and IDT staff on documentation requirements and best practices (Willingness to Learn and Teach)
Serve as a resource for clinical documentation, regulatory guidance, and survey preparation
Promote a culture of accountability, accuracy, and teamwork (Truthful & Caring)
Required Qualifications
Active RN or LPN license in New Mexico
Minimum 2 years’ nursing experience, preferably in long-term care
MDS experience preferred; strong understanding of MDS 3.0, PDPM, and RAI process required
Knowledge of Medicare/Medicaid reimbursement and Quality Measures
Experience with audits, reporting, and regulatory compliance preferred
Proficient in EHR and MDS software systems
Strong organizational, analytical, and communication skills
Ability to work independently while collaborating across departments
High attention to detail and commitment to clinical accuracy
