Responsibilities
Complete and maintain thorough documentation including but not limited to monthly notes and reports, 14‑day plans, 30‑day plans, 45‑day plans, social histories, quarterly reports, and annual reports.
Assess the needs of foster children and implement tangible objectives, strategies, and services to address all identified needs.
Complete home and field visits multiple times a month for each child.
Attend or coordinate meetings on behalf of foster children and foster families, including FAPT, court, FPM, treatment teams, and VEMAT meetings.
Supervise visitations between foster children and their birth families, provide transportation for identified services, and make service referrals to various agencies.
Participate in ongoing trainings required by LCPA standards, HumanKind, and supervisor.
Participate in all audits, including internal and external conduct by LCPA Licensing Inspector.
Use Placement/Matching Protocol. In the event an assistant is needed for placement, work with the Program Manager.
Provide ongoing support and training to foster families.

Requirements
Effectively communicate professionally and appropriately, both written and verbally, to a diverse range of people.
Ability to work effectively and respectfully in a team environment with a diverse group of coworkers, program staff, and foster children/families.
Ability to self‑direct, self‑motivate, and demonstrate non‑judgmental professional decision‑making.
Ability to manage multiple projects and various deadlines simultaneously.
Possess strong organizational skills and understand the importance of details.
Ability to handle and manage crisis situations appropriately.
Maintain confidentiality.
Comply with LCPA standards, HumanKind and HK TFC policies and procedures, and remain up to date on any changes.
Baccalaureate degree in any field plus two years of experience in providing casework services to children and families; OR a baccalaureate degree in social work or a related field (sociology, psychology, special education, or counseling) and one year of experience; OR a doctorate or master’s degree in social work accredited by the Council on Social Work Education or a related field with a student placement or one year of experience.
Valid driver’s license and acceptable driving record.
One year of case management experience, preferably with children and families.
Complete, pass, and maintain all required background checks and drug tests.
Must be able to respond to on‑call emergencies within an approved amount of time as determined by Program Manager.
