Overview

Upper Valley Medical Center provides direct and indirect clinical social work and discharge planning services for patients and their families in various inpatient and outpatient settings. Participates as a professional team member in patient care planning along with other hospital and community agency personnel.

SOCIAL SERVICES - MSW
PRN / VARIED HOURS / 8 HOURS PER PAY

Provides direct and indirect clinical social work and discharge planning services for patients and their families in various inpatient and outpatient settings. Participates as a professional team member in patient care planning along with other hospital and community agency personnel.
Frequent contact with patient care services staff, physicians, and ancillaries to discuss patient care and diagnostic needs. Participate in care rounds and other interdisciplinary collaboration within the in-patient and outpatient programs. Frequent contact with community agencies, third party payers, outside review organizations and case managers.

Qualifications

Master’s in Social Work
Licensure/Certification/Registration: LSW required; LISW preferred
Experience: Minimum 1 - 3 years of job-related experience. Preferred: Previous hospital or community social work experience; knowledge of community agencies
Other requirements: Excellent interpersonal skills, critical thinking and written communication; excellent organizational skills; ability to work in a busy, stressful environment
