Overview
Social Worker – Case Management (Travel)
| Everett, WA | Start May 26, 2026 | 8 weeks | $1,586–$1,642 per week | Guaranteed hours: 72 hours per 2 weeks | Shift: 4x10-hour days | Short-Term Acute Care inpatient medical social work / case management for adult and geriatric populations. This is a travel, temporary assignment in a hospital setting.
Responsibilities
Conduct biopsychosocial assessments and needs assessments for adult and geriatric patients; participate in discharge planning and care coordination
Develop and maintain individualized plans of care; coordinate referrals to home health, DME, rehabilitation, and guardianship as appropriate
Document in EPIC; develop and update the plan of care and progress notes; ensure compliance with Medicare/Medicaid processes
Collaborate with an interdisciplinary team (PT, interpretation services, social services, pharmacy, transportation) to support patient goals
Manage a caseload of approximately 18–24 patients, with weekends up to ~30; participate in unit operations and float across similar units and campus locations as needed
Provide patient and family education regarding discharge instructions and access to community resources
Requirements
Active Washington license in one of the following: LICSW or LSWAIC; or LASW or LSWAAS; or current WA RN license for RN applicants. Washington license in hand required
Certifications: BLS required; RN licensure required for RN applicants
Education: Master’s degree in Social Work (MSW) or RN with clinical background; LMSW/LCSW eligible per license
Experience: Minimum 1 year of clinical experience; acute care experience with discharge planning / care coordination; strong care management background across Med-Surg, LLOS, and ED preferred
Skills: Proficiency with EPIC charting; ability to work with adult/geriatric populations; strong communication and collaboration with internal/external teams
Additional: First-timers accepted; ability to float across like units and between campus locations; pre-start modules (~16 hours) and 40-hour orientation; willing to work holidays as required
