Responsibilities

Conduct psychosocial assessments for adult and geriatric inpatients
Develop and implement discharge planning and care coordination plans
Facilitate referrals to home health, rehabilitation, long‑term care, and related services
Collaborate with an interdisciplinary team (PT, Pharmacy, Interpretation, Social Services, Transportation) to support patient care
Document assessments, plans, and progress in EPIC; maintain accurate care documentation
Provide patient and family education, counseling, and crisis intervention as needed
Advocate for patient rights and coordinate smooth transitions of care to appropriate services
Requirements

Active Washington state social work license (LMSW/LCSW accepted; LICSW/LSWAIC/LASW/LSWAA acceptable per posting)
Pending license accepted with WA license in hand at submittal per client guidance
Master’s degree in Social Work (MSW) or equivalent
Minimum 1 year of relevant experience in acute care, discharge planning, or care coordination
BLS certification
Ability to work in an inpatient acute care setting and to float within the hospital system
EPIC charting experience preferred
