Why does the job exist?
This position provides effective advocacy for long‑term care residents and acts as a liaison with internal and external entities, ensuring resident rights are protected and that resident and family concerns are addressed. It also provides comprehensive resident assessments, care planning, and discharge planning.

How does it get done?

Advocates and assists resident to attain or maintain the highest level practicable well‑being.

Performs medically related social services needs in accordance with State, Federal and Veteran Regulations.

Completes comprehensive resident assessments and care plans.

Documents resident needs, requests, appointments and other necessary information in the computer system.

Reviews grievances and resident/family concerns and takes appropriate steps toward resolution.

Assesses for trauma, develops care plans to meet residents’ needs, and monitors for PTSD symptoms.

Participates in the pre‑admission assessment process.

Facilitates the discharge planning process with resident and interdisciplinary team members.

Participates in the psychopharmacology review team to assess resident emotional, mental and cognitive impairments and related medication.

Who are the customers?
Residents of a long‑term care facility; residents admitted for short‑term skilled care with an expectation of discharge back to the community. Work with families and/or legal representatives, insurance care coordinators, community medical providers, community therapy providers, medical equipment providers and veteran's services.

Ideal Candidate

Ability to read, write, speak, and understand the English language.

Ability to deal tactfully with personnel, residents, family members, visitors, government agencies, and the general public.

Ability to function independently, with flexibility, personal integrity, and to work effectively with residents, personnel, and support agencies.

Must maintain current New Mexico Social Work License.

Minimum Qualification
Associate’s Degree in Human Services or Social Sciences and two (2) years experience in mental health and/or substance abuse counseling. Any combination of education from an accredited college or university in a related field and/or direct experience in this occupation totaling four (4) years may substitute for the required education and experience.

Employment Requirements
This position is designated as safety sensitive. Employment is subject to pre‑employment and random testing in accordance with federal and state law, rules and regulations regarding alcohol and/or drug testing, tuberculosis testing, and a criminal background check pursuant to the New Mexico Caregivers Criminal Screening Act.

Working Conditions
Work in office area(s) and throughout the nursing service area, including nurses’ stations and resident rooms. Moves intermittently during working hours, is subject to frequent interruptions, and may encounter hostile or emotionally upset residents, family members, personnel, and visitors. Requires communication with medical staff, nursing personnel, and department directors. May work beyond normal hours, including weekends and holidays when necessary. Must be able to lift or move at least 25 pounds to a height of 5 feet and a distance of 50 feet. Additionally, employees may be required to comply with COVID‑related requirements prescribed by CMS.
