We are seeking an experienced MDS Nurse (RN) to join our team at River Bend Health and Rehabilitation, a premier post‑acute care provider in Asheville, NC. As a vital member of our interdisciplinary team, you will play a key role in ensuring the highest quality of care for our residents.
MDS Nurse Job Responsibilities
Conduct thorough assessments of residents' medical, social, and psychological needs, using the Minimum Data Set (MDS) system to identify and address concerns
Develop and implement Individualized Care Plans, in collaboration with other healthcare professionals, to promote optimal resident outcomes
Provide direct patient care, including administering medications, wound care, and other treatments as needed
Maintain accurate and detailed records of resident assessments, care plans, and treatment progress
Participate in interdisciplinary team meetings to discuss resident care and coordinate services
Collaborate with families and caregivers to ensure open communication and build trust
MDS Nurse Requirements
Current RN licensure in the state of North Carolina
Bachelor's degree in Nursing
Minimum of 2 years of experience in post‑acute care or long‑term care
Strong assessment and critical thinking skills
Excellent communication and interpersonal skills
Ability to lift, transfer, and reposition residents as needed
