Job Title: Social Service Director – Full-Time

The Greens at Pinehurst is seeking a compassionate and dedicated
Social Service Director
to join our team. In this role, you will support residents by identifying and addressing their
psychosocial, emotional, and mental health needs , while connecting them with the resources and services necessary to enhance their overall well-being.
This position plays a vital role in promoting a positive environment where residents can maintain their
individuality, independence, and dignity , while achieving the highest possible quality of life.
Key Responsibilities:
Complete
admission assessments
and participate in
discharge planning
Assess and document residents’ psychosocial needs and progress
Collaborate with the
interdisciplinary team
to support resident care and quality of life
Ensure compliance with
state and federal regulations
related to social services
Participate in
care plan meetings
and quality assurance initiatives
Maintain and utilize a directory of
community resources
for residents and families
Advocate for and protect
resident rights , ensuring a safe and supportive environment
Provide guidance and support to residents, families, and staff
Assist in developing and implementing social services policies and procedures
Help prevent and address concerns such as
abuse or neglect , in accordance with regulations
Qualifications:
Bachelor’s degree in
Social Work
OR a related human services field (e.g., sociology, psychology, gerontology, rehabilitation counseling, special education)
Minimum of
1 year of experience in a healthcare setting
working directly with individuals
Strong communication, organizational, and interpersonal skills
Passion for improving the lives of residents in a long-term care setting
