Position:
Health Caseworker
Note:
The Health Caseworker will address barriers to safety and wellness for especially vulnerable clients and provide strengths-based, client-centered case management to various refugee populations. Responsibilities include developing service plans that meet individual client needs and preferences using a broad range of services and resources in accordance with client eligibility and program requirements.
Major Responsibilities
Work within the Casework team in IRC San Jose to ensure that clients receive health care services, enrollment, follow up care, and referrals.
Schedule clients for Refugee Health Clinic (RHC) appointments and ensure access.
Conduct a needs assessment to review health care needs for clients.
Devise a Plan of Care in collaboration with each client to establish goals for the six-month service period.
Perform a simple mental health assessment with each client aged 14 and above.
Schedule health care appointments for clients.
Assist clients in navigating health care services, including OB-GYN follow up, dental care, and mental health services.
Provide clients with extended orientation on health care systems, insurance enrollment, and navigating transportation systems.
Promote client self-sufficiency by guiding clients in accessing system resources independently.
Maintain up-to-date documentation in client's case file to ensure services are properly documented throughout the client's service period; document all services provided in IRC’s database, Efforts to Outcomes (ETO).
Attend relevant trainings and meetings at the local and national level, including mandatory meetings with program funders.
Participate in funder site visits/monitoring activities.
May train and lead the activities of volunteers and interns.
Other duties as assigned.
Job Requirements
Education:
Bachelor’s degree in social work, public health, or related field of study preferred.
Work Experience:
Minimum of one year of case management experience with similar populations and/or client needs required. Background in health/mental health services is highly desirable.
Demonstrated Skills & Competencies:
Strong relationship building, diplomacy, and networking skills; ability to effectively build internal and external relationships. Demonstrated success working and communicating effectively in a multi-cultural environment. Self-starter with excellent problem-solving skills combined with the proven ability to prioritize and manage time effectively. Attention to detail and accuracy in work product. Fluent in English required, both spoken and written; proficiency in Dari, Pashto or Spanish is desired. Proficient in Microsoft Office applications (Word, Excel, Outlook); ability to use the internet and other digital tools for data entry, research, and problem-solving purposes. Valid driver’s license and access to a personal, insured vehicle.
Working Environment
A combination of standard office environment, remote work, and ‘field’ time within the service delivery area to perform the above outlined responsibilities.
May require occasional weekend and/or evening work.

This position is represented by the OPEIU union and subject to the terms and conditions of a collective bargaining agreement.
Compensation:
Pay Range: $26.00 - $27.00. Posted pay ranges apply to US-based candidates. Ranges are based on factors including labor market, job type, internal equity, and budget. Exact offers are calibrated by work location, individual candidate experience and skills relative to the defined job requirements.
