Social Worker MSW Full-time Day

The Social Worker provides comprehensive services to include psycho-social assessments, community resource information and referrals to patients, their families and loved ones, ranging in age from infants through geriatrics, as it supports the continuum of care. Provides coordination of complex discharge planning needs, and facilitates multidisciplinary care team involvement to expedite plan of care for patients.

Required Qualifications

Master's degree in social work from an accredited CSWE program.

1 year of Social Work experience in any healthcare setting.

1 year of experience in an acute care setting or successful completion of TIPs program or Case Management Orientation Program.

Preferred Qualifications

Upon hire: Clinical Social Worker License (LCSW), or license eligible.

Basic understanding of acute medical surgical healthcare terminology and prior case management experience.

Compensation
Pay Range: $43.63 – $65.70 (base pay). Additional compensation may be available, such as shift differentials, standby/on-call, overtime, premiums, extra shift incentives, or bonus opportunities.
