RN / LPN MDS Coordinator

Schedule:

Full-Time | Monday–Friday, 8:00 AM – 4:30 PM

|

Pay Rate:

$36.00–$46.00 per hour

Responsibilities

Coordinate and complete all MDS assessments in accordance with CMS and state regulations

Ensure accurate implementation of RAPS, triggers, and care planning processes

Maintain and update MDS and care plan policies and procedures

Monitor and manage assessment schedules to ensure timely submission

Track Quality Indicators (QI) and Quality Measures (QM) and implement corrective actions as needed

Collaborate with the interdisciplinary team, including nursing, therapy, social services, and dietary

Assist with discharge planning by coordinating care plan components

Participate in facility surveys, audits, and inspections

Support quality assurance and performance improvement initiatives

Facilitate interdisciplinary care plan meetings

Communicate effectively with residents, families, and external care providers

Evaluate residents’ functional abilities to support therapy and activity planning

Qualifications

Active Florida RN or LPN license (required)

Minimum 2 years of nursing home experience preferred

Strong knowledge of MDS, PPS, QI/QM, and care planning processes

Excellent organizational, communication, and teamwork skills

High level of professionalism, integrity, and accountability

Resident-focused mindset with strong problem-solving abilities
