Job Description
The Social Worker is responsible for improving outcomes for patients and families by addressing complex psychosocial and economic needs. This role involves providing education and consultation on available resources, collaborating with the interdisciplinary team to develop discharge plans, and facilitating and monitoring their implementation.
The Social Worker assists patients and families in coping with illness, diagnosis, treatment options, and prognosis. Using advanced practice skills, this role mobilizes community resources, reduces patient risk, serves as a liaison between healthcare services and the community, and provides crisis intervention when needed. A strong commitment to patient- and family-centered care is essential.
Responsibilities
Assess and manage the psychosocial and economic needs of patients and families
Collaborate with care teams to develop and implement discharge plans
Provide education on community and healthcare resources
Assist patients and families in adjusting to illness and treatment
Deliver crisis intervention services as needed
Promote patient- and family-centered care
Maintain accurate and timely documentation
Requirements
Minimum 2+ years of Social Worker experience (Required)
Master’s degree in Social Work from an accredited institution (Required)
Experience with OB population (Required)
