Position Summary
This role offers a $5,000 New Hire Bonus.
Candidate must be located in Miami‑Dade County (Hialeah, 33010‑33018).
Role will require 50‑75% travel for face‑to‑face visits in Miami‑Dade County.
Schedule: Monday‑Friday, 8:00 am‑5:00 pm.
We are seeking a self‑motivated, energetic, detail‑oriented, highly organized, tech‑savvy Case Management Analyst to support and coordinate the care of our members. Responsibilities include telephonic and/or face‑to‑face assessment, planning, implementation and coordination of case management activities; managing a caseload of supportive and medically complex members; developing proactive plans to enhance short and long‑term outcomes; determining appropriate services and supports such as prior authorizations, coordination with PCPs and specialized providers, condition management information, medication reviews, and community resources.
Required Qualifications
Bilingual (English & Spanish).
1+ year of experience in Behavioral Health or Long‑Term Care.
Computer proficiency in Microsoft Word, Excel, Outlook.
Must reside in Miami‑Dade County.
Willing and able to travel 50‑75% of time for face‑to‑face visits in Florida.
Preferred Qualifications
Ability to multitask, prioritize and adapt to a fast‑paced environment.
Effective communication skills, both verbal and written.
Managed Care experience.
Case Management and Discharge Planning experience.
Education
Bachelor’s degree required, preferably in Social Work or a related field.
Hours & Compensation
Anticipated weekly hours: 40. Full time. Pay range: $21.10 – $40.90 per hour. Eligible for bonus, commission or short‑term incentive.
