Healthcare Consultant I Compensation $34.13 per hour Benefits are available to full-time employees after 90 days of employment 401(k) with company match is available after 1 year of service (on eligibility dates) Healthcare Consultant I Highlights Schedule: Monday–Friday, 8:00 AM – 5:00 PM (standard business hours) Work setup: Work from home, with heavy field work Travel: Approximately 75% travel for face-to-face member visits within Miami-Dade County (mileage reimbursed to/from member homes) Interview required Must be fluent in English and Spanish Healthcare Consultant I Responsibilities Coordinate case management activities for Medicaid Long Term Care/Comprehensive Program members Conduct comprehensive evaluations/assessments using care management tools and data review Develop, implement, and monitor assigned care plans and case management activities Coordinate with PCPs, skilled providers, and multidisciplinary teams to support member outcomes Identify and escalate quality-of-care concerns through established channels Use negotiation skills to secure appropriate services/supports to meet member health needs Use motivational interviewing/influencing skills to drive member engagement and behavior change Provide coaching, education, and support to help members make informed healthcare decisions Document and track care management activity in compliance with regulatory/accreditation guidelines and company policies Healthcare Consultant I Requirements High School Diploma or GED (verifiable) Attendance is mandatory for the first 90 days Must reside in Miami-Dade County, FL in one of these zip codes: 33142, 33133, 33134 Must be able to complete telephonic + field visits (home/assisted living
