Healthcare Consultant I (Case Management) | Miami‐Dade County, FL Healthcare Consultant Overview: Pay: $32-34/hour Schedule: Monday–Friday | 8:00 AM–5:00 PM Location Requirement: Miami‐Dade County, FL Travel: Approximately 75% local travel for in-person visits ( mileage reimbursed to/from member homes) Ready to apply or have questions? Contact: Ally Boelstler Call/Text: 586-710-7958 What you’ll do: Coordinate case management activities for Medicaid LTC/Comprehensive Program members Complete telephonic and in-person assessments using care management tools and data review Create and support care plans—implement activities, track progress, and document outcomes Coordinate with PCPs, skilled providers, and community resources to ensure appropriate services/supports Support members with education, coaching, and engagement (including motivational interviewing techniques) Identify/escalate quality-of-care concerns through established channels Ensure documentation is compliant with regulatory/accreditation requirements and company policies Required for Healthcare Consultant: Fluent in English and Spanish Comfortable with field-based member visits (home/ALF/SNF settings) and managing a caseload Microsoft Office proficiency (including Excel ) Verified High School Diploma/GED Must reside in Miami‐Dade County (target zip codes: 33142 / 33133 / 33134 ) Preferred: Case management experience ( required per hiring team) Long term care experience (strong plus) Strong organization, multitasking, and communication skills in a fast-paced environment Education: Bachelor’s degree required (social work or related field preferred) Ready to apply or have questions?
