
Sr. DSP Medical Coordinator

The Sr. DSP Medical Coordinator is responsible for all of the duties of a Direct Support Professional plus scheduling, tracking and documenting all medical appointments, as well as scheduling all labs and required tests.

Responsibilities
Document all paperwork required, including incident reports, log entries, and Res. Hab. progress notes when appropriate.
Track all medical appointments and follow-up appointments and document in tracking forms.
Track all lab tests and other tests, ensuring correct documentation in medical tracking log.
Document all consults, refusals and MD recommendations appropriately.
Ensure medication inventories are completed at least weekly, order new medication as prescribed in a timely manner, and maintain the medication storage area in accordance with regulations.
Identify medication errors and communicate the errors to the nurse and manager effectively.
Communicate with the nurse and manager via a Medical Communication Log.
Follow all AMAP procedures.
Attend ISP or other meetings when requested.
Maintain open lines of communication with management and staff.
Qualifications
NYC Driver license or driver license required.
HS Diploma or GED required.
1+ year of HHA experience required, CNA experience a plus.
2+ years of Direct Support Professional experience a plus.
