Case Manager

CommQuest is seeking Full-Time Case Manager to join our Canton Campus in Canton, Ohio.
Case Managers (CM) render services to individuals 18 and over focusing on recovery and resiliency principles as they empower individuals experiencing substance use disorders to integrate, function, and succeed in their natural environments. The Case Manager will assess, plan, implement, coordinate, monitor, and evaluate the options and services necessary to meet the needs of clients. The CM also provides symptom monitoring, crisis management, advocacy, and connections to natural and community supports and resources.
Case Managers are expected to have the ability to establish working relationships with diverse individuals in a variety of settings and situations and must demonstrate an ability to work collaboratively with others on behalf of the client both within the agency, in the community, and/or family setting. The position requires experience working with adults with mild to moderate mental illness and substance abuse disorders.

What you'll do:
Empowerment and promotion of recovery and resiliency principles.
Facilitation, education, and development of sober living skills.
Provides advocacy and outreach on behalf of clients.
Provides client and family education demonstrating competency in clinical progress notes and supervisory observation.
Links clients with referral services, employers, family members, other providers, and any/all necessary resources to assist the client in obtaining goals.
All other duties as assigned
Schedule:
Full-time, Dayshift, some evenings may be required.
Minimum Qualifications:
A Bachelor's degree in Social Work, Psychology or a related Human Service field is highly preferred; minimum requirement of high school diploma or GED.
Chemical Dependency Counseling Assistant (CDCA) through the Ohio Professionals Board, required.
Previous experience working with adults with severe mental illness and substance use disorders, highly desired.
Must possess strong communication skills, both verbally and written, and work collaboratively with others on behalf of the client both within the agency and in the community.
Possesses a valid driver's license, vehicle insurance, and a driving record that is in compliance with agency insurance requirements.
