Day Shift, 8:00am to 4:30 pm
Job Description
Perform comprehensive patient assessments (vital signs, pain, intake/output, mental status)
Monitor and report changes in patient condition to RN/MD
Assist with care planning and goal setting with patients and families
Conduct follow-up calls, discharge support, and patient education
Coordinate care and services in home health settings
Maintain accurate documentation, medication reconciliation, and specimen labeling
Administer medications via oral, IM, SQ, NG/G-tube, eye/ear drops
Position Requirements
Strong communication skills with patients, families, RNs, and physicians
Ability to work independently in home and community settings
Critical thinking and sound clinical judgment
Effective time management and organizational skills
Comfortable traveling between patient homes
Team-oriented mindset
