Job Purpose
The UBHCC Nurse is responsible for supporting the multidisciplinary crisis team by delivering coordinated, high-quality care to clients at the Urgent Behavioral Health Crisis Center.
Qualifications
Must be a licensed LPN.
Experience in psychiatric or behavioral health care and healthcare technology is preferred.
Ability to conduct basic nursing assessments.
Core Job Duties
Obtain and record vital signs.
Perform blood and urine collection as requested by prescribers.
Reconcile home medications.
Complete triage assessments.
Based on triage assessment outcomes, determine appropriate care pathways and facilitate timely engagement with the next provider to ensure efficient service delivery.
Monitor clients’ physical and behavioral health status during their visit.
Communicate relevant information to the clinical team.
Conduct violence risk screening using the agency screening tool.
Review information from PDMP (Prescription Drug Monitoring Program).
Facilitate telehealth connections for Nurse Practitioner evaluations.
Call in prescriptions and refill medications as directed by prescribers.
Complete documentation in a timely manner.
Communicate information orally in an effective and accurate manner.
Assure program operates with the constraints of the agency’s certification (CARF, DMH) standards and strategic plan related to clinical care.
Abide by program and agency policies and procedures.
Other Duties: Participate in special projects and agency initiatives as assigned.
