Job Description
Responsible for providing short-term psychotherapeutic services including individual, marital, family and group treatment.
Principal Accountabilities
Develop written individual recovery plans.
Engage to enhance functioning, effective coping, illness management, self-management, wellness and quality of life.
Partner with patient's family, peers, para-professionals and professionals in supporting, monitoring and stabilizing consumers in the community.
Participate in community events, human service meetings as appropriate or mandated.
Provide case management services.
Develop and maintain rich collaborative working relationships with hospital emergency departments, service providers, and agencies.
Maintain timely and complete chart documentation.
Incorporate use of criteria sets in context of utilization management.
Perform other related duties as assigned.
Qualifications
Master's degree in social work.
Previous clinical or counseling experience.
LCSW or LSW or CSW license.
