Practice Setting: Outpatient Hematology/Oncology Reason for Coverage: New service line Dates of Coverage: March 2026 - March 2027 Patient Population for Required Coverage: Adult, Geriatric What Percentage of the Practice is Hematology %, Oncology %, Internal Medicine %?: Hematology 50%; Oncology 50% - 0% IM Average Number of Patients Per Day - OUTPATIENT (Clinic/Office): 30 average Average Number of Patients Per Day - HOSPITAL (Call/Rounding): 0 Clinical Area: Oncology - Outpatient Setting Documentation System/EMR: EPIC Estimated Credentialing Timeframe: With an IN license 60 days; without IN license 90 days Facility will reimburse for the following costs: Flight, Hotel, Rental Car, Mileage Billed at the IRS Standard Rate

Required Procedures:

• Mgmt. of VAD/CV Catheters
• Bone Marrow Biopsy

Required Duties:

• Participation on a Tumor Board
• Mgmt. of Bone Marrow Transplants
• Orders Tests
• Blood Bank
• Orders Images
• Prescribes

Administration of Chemotherapy:

• Physician
• Nurse oncologist

Submission Requirements

• Board Certification (provider must have AMBS/AOA board certification in one, and competency in the other demonstrated) - REQUIRED
• Active IN State License or IMLC with LOQ in hand - REQUIRED
• BLS - REQUIRED
