Health System is looking for an Outpatient long term Hematology/Oncology provider who has interest in going perm and helping build the program to start ASAP!
Practice Setting: Outpatient Hematology/Oncology
Coverage will include two locations
Reason for Coverage: New service line
Dates of Coverage: ASAP - ongoing
Patient Population for Required Coverage: Adult, Geriatric
What Percentage of the Practice is Hematology %, Oncology %, Internal Medicine %?: Hematology 50%; Oncology 50% - 0% IM
Average Number of Patients Per Day - OUTPATIENT (Clinic/Office): 30 average
Average Number of Patients Per Day - HOSPITAL (Call/Rounding): 0
Clinical Area: Oncology - Outpatient Setting
Documentation System/EMR: EPIC
Estimated Credentialing Timeframe: With an IN license 60 days; without IN license 90 days
Required Procedures:
- Mgmt. of VAD/CV Catheters
- Bone Marrow Biopsy
Required Duties:
- Participation on a Tumor Board
- Mgmt. of Bone Marrow Transplants
- Orders Tests
- Blood Bank
- Orders Images
- Prescribes
Administration of Chemotherapy:
- Physician
- Nurse oncologist
- Board Certification (provider must have AMBS/AOA board certification in one, and competency in the other demonstrated) - REQUIRED
- Active IN State License or IMLC with LOQ in hand - REQUIRED
- BLS - REQUIRED
- Provider must be willing to commit to long term assignment (Minimum of 6 months) - REQUIRED
