MercyOne Dyersville Senior Care is a 40-bed senior care facility providing high-quality care to residents. We offer a full range of services in a personal, home-like setting. Our facility adjoins MercyOne Dyersville Medical Center, allowing immediate access to medical care. We offer the following long-term care services.
Services
Professional nursing care 24/7
Emergency department, family practice and pharmacy on-site
Medicare Part B rehabilitation program
Healthy meals and registered dietitians to monitor nutritional needs
Social worker to provide counseling, guidance, and emotional support
Role:
MercyOne Dyersville Senior Care — MDS Nurse Coordinator. The MDS Nurse Coordinator conducts and coordinates the resident assessment in accordance with state requirements and facility policies.
Responsibilities
Act as a collaborative member/leader of the healthcare team, practicing the art and science of nursing as prescribed by applicable standards.
Lead the resident assessment (MDS) process, including RAPs/CAA and Triggers/CATs, and coordinate interviews and data evaluation for each resident.
Develop preliminary and comprehensive assessments of nursing needs, utilizing required forms and policies.
Coordinate the RAI process and ensure timely documentation and completion within required time frames.
Develop written plans of care for each resident, identifying problems, care to be given, goals, and responsible professionals; ensure measurable objectives and timetables.
Ensure initial resident assessments are completed timely and comprehensive care plans are filed in the chart.
Ensure care is delivered in accordance with residents’ wishes, including advance directives.
Assist with monitoring documentation compliance and provide education on proper documentation.
Maintain confidentiality of all resident care information and ensure privacy in nursing care.
Support and mentor colleagues across departments; address issues through constructive dialogue and notify the department manager of major concerns.
Function independently, with the ability to make independent decisions and work effectively with residents and staff.
Report occupational exposures per community policies and procedures.
Education & Qualifications
Bachelor of Science in Nursing (BSN) preferred or within 4 years of hire.
Must hold a current, active registered nurse license in the state of Iowa.
Knowledge of nursing and medical practices, laws, regulations, and long-term care guidelines.
AANAC certification or must obtain within 2 years of hire.
Experience in rehabilitative/restorative nursing and long-term care or geriatric population preferred.
Strong customer service, professionalism, and interpersonal skills; ability to adapt to change in a fast-paced environment.
Proficiency with Microsoft Word, Excel, PowerPoint; familiarity with email and calendars; ability to type with speed and accuracy.
Ability to plan, organize, develop, implement, and interpret programs, policies, and procedures for the organization.
Mandatory training: Child and Dependent Adult Abuse; Basic Life Support (BLS) within 30 days of hire and every 2 years thereafter.
Schedule
Full time 40 hours per week
