Overview
Transitions At Home , is currently seeking to hire a Full-Time, compassionate and experienced
Physical Therapist
to join our growing home health team. This position offers the opportunity to provide personalized care in a home setting, ensuring patients receive the highest quality of service and support throughout their recovery journey.

Your Role
As a Home Health Physical Therapist, you’ll deliver skilled therapy services based on individualized care plans and physician orders. You’ll work closely with the interdisciplinary team to support patients in regaining strength, mobility, and independence at home.
Key Responsibilities
Complete OASIS assessments, evaluations, and progress notes
Develop and revise care plans
Perform skilled physical therapy procedures
Educate patients and families on home programs and daily activities
Collaborate with physicians and the care team
Participate in IDT meetings, in-service training, and QA initiatives
Ensure accurate documentation and use of ICD-10 codes
Qualifications
Active Wisconsin Physical Therapist license
CPR certification
Valid WI driver’s license & car insurance
Two (2) years of experience preferred
Comfortable working independently in a home care environment
