Transitions At Home
is hiring a Full‑Time Physical Therapist Assistant to help patients regain independence in their homes. The assistant administers physical therapy per physician orders and under the direction of the Administrator/Clinical Manager.
We offer a flexible schedule, independence, supportive clinicians, and one‑on‑one patient relationships for compassionate care.
Hours
8:00am – 4:30pm
Monday through Friday
No holidays

Qualifications
Active Wisconsin Physical Therapist Assistant License
Preferred: 2 years of experience
CPR certification
Valid Wisconsin driver’s license and car insurance
