Position Summary

POSITION SUMMARY:

The Advanced Practice Professional (APP) coordinates and delivers clinical services for the comprehensive management of primary care and behavioral medicine patients. The APP conducts biological, social and psychological assessments, collaborates with specialty teams, reviews and directs plans of care, performs diagnostic and therapeutic procedures, coordinates discharge planning, and participates in resident and student education.
Responsibilities

Manages comprehensive, individualized care for primary care and behavioral medicine patients and their families in an ambulatory setting.
Conducts consults, pre‑procedure evaluations, acute visits, and wellness visits.
Completes integrated assessments and orders additional tests or consults as needed.
Performs diagnostic tests, communicates results, and consults with clinic, hospital staff on abnormal findings.
Documents comprehensive clinical evaluations in the medical record.
Coordinates patient follow‑up services and tests, and develops individualized plans of care, including teaching, follow‑up planning, and necessary referrals.
Identifies unstable or complex patient situations and facilitates appropriate intervention.
Responds to emergency situations and initiates effective emergency care.
Assists in collection of outcome measures and demonstrates critical thinking and diagnostic reasoning in clinical decision making.
Utilizes equipment safely and bases decisions on scientific principles, standards of care, and research findings.
Provides education to nursing staff, hospital staff, students, patients, families, and the community using appropriate teaching strategies.
Promotes a learning environment that responds to patient preferences and readiness to learn.
Updates own knowledge base throughout the year and completes all identified clinical competencies.
Serves as a resource for staff caring for primary care patients and provides educational programs to professionals and paraprofessionals.
Participates in community programs and distributes literature as appropriate.
Documents plans, interventions, and evaluations clearly for each patient visit following hospital policies and compliance standards.
Collaborates with leaders and staff to achieve patient outcome improvements in care.
Leads development and implementation of changes in primary care practice that positively impact patient outcomes.
Supports creative problem‑solving and provides professional support to medical and nursing staff.
Participates in and initiates interdisciplinary approaches to patient care, program development, and education, serving as a role model.
Promotes and practices innovation of the expanded role in patient delivery of care throughout inpatient and ambulatory settings.
Provides leadership for committees and task forces, documents hours, submits billing reports, reviews primary care data from patient registries, reports, and suggests changes.
Participates in clinical trials and research studies with the Principal Investigator and assists faculty, administrators, and directors in collecting outcome measures.
Performs any other duties or projects as assigned.
Minimum Qualifications for Nurse Practitioner

Graduate of a CCNE accredited nursing program with a Master’s Degree in Nursing.
National Certification from a recognized APRN Certification Center.
Licensure or eligibility for licensure as a Registered Nurse and Advanced Practice Registered Nurse in the state of practice (Maryland).
BLS Certification required; additional Advanced Life Support training at the discretion of the department.
Experience for Nurse Practitioner

Primary care experience preferred.
Eligible new graduates will be considered.
Preferred Qualifications for Nurse Practitioner

ACNPs and FNPs considered.
Minimum Qualifications for Physician Assistant

Bachelor’s Degree or Master’s Degree in a Medical/Surgical PA program required.
Graduate of an ARC‑PA approved PA training program required.
Certification by the National Commission for Certification of Physician Assistants required.
Must be licensed or eligible for licensure by the Board of Medicine in the state of practice.
BLS Certification required; additional Advanced Life Support training at the discretion of the department.
Experience for Physician Assistant

Primary care experience preferred.
Eligible new graduates will be considered.
Education, Certification and Licensure for Physician Assistant (Preferred)

Master’s Degree as a Physician Assistant preferred.
Physical Requirements

Work requires strength and/or stamina for lifting, moving, stooping, reaching, positioning patients, standing, walking, and carrying patients, materials, and equipment weighing 40+ pounds.
Working Environment

Working closely with others.
Protracted or irregular hours.
Working around biohazards and infectious diseases.
Working around the deceased.
Working with hands in water.
Electrical hazards associated with patient care equipment.
Skills and Abilities

Proficiency with computers.
Strong communication skills.
