Job Summary
We are looking for an LVN Clinical Intake Coordinator, you will serve as a critical connection point between patients, families, physicians, and care teams ensuring seamless transitions into home health services and timely coordination of care.
Responsibilities
Manage and coordinate incoming referrals, calls, and inquiries from patients, families, physicians, and healthcare partners
Verify insurance coverage and ensure accurate intake processing
Coordinate home health referrals and ensure timely patient onboarding
Track hospitalized patients and facilitate smooth transitions into agency care
Communicate effectively with nursing, therapy, social work, and physician teams
Assist with scheduling and staffing coordination based on program needs
Support operational workflows and ensure continuity of care across disciplines
Serve as a working coordinator for intake processes and workflow efficiency
Experience/Qualifications
Active LVN (Licensed Vocational Nurse) certification
Minimum 1 year of healthcare experience
High School Diploma or GED
Preferred
Associate’s or Technical Degree (or equivalent experience)
3+ years of healthcare experience, ideally in home health, intake, or care coordination
