
LTSS Service Care Coordinator

This role is work from home with 50–75% travel within the Illinois counties of: Jo Daviess, Carroll, Whiteside, Rock Island, Mercer, Henry, and Bureau; mainly the Quad Cities area. The position supports Medicaid LTSS waiver members who are aging and physically disabled, with approximately 90% of work occurring in the community and 10% in supportive living settings.
Applicants must hold a bachelor’s degree or be an RN or LPN licensed in Illinois, as required for the education and experience criteria below. Work schedule is Monday–Friday, 8 AM–5 PM Central Time Zone. Mileage is reimbursed at the current IRS guideline rate for round‑trip travel.
Position Purpose
Evaluates the needs of the member, the resources available, and recommends and/or facilitates the best plan for optimal outcomes.
Assists with developing ongoing long‑term care plans and identifies providers, specialists, and community resources needed.
Coordinates between members, families or caregivers, and the care provider team to ensure access to identified services.
Provides resource support to members and families for needs such as employment, housing, participant direction, independent living, justice, and foster care based on service assessment.
Monitors care plans and member status, providing recommendations to adjust plans as needed.
Interacts with long‑term care healthcare providers and partners to ensure member needs are met.
Collects, documents, and maintains member information and care management activities to ensure compliance with state, federal, and third‑party payer regulations.
May perform on‑site visits to assess member needs and collaborate with providers or resources.
Facilitates education to members and families on procedures, provider instructions, service options, referrals, and benefits.
Provides feedback to leadership on opportunities to improve and enhance quality of care and service delivery cost‑effectively.
Performs other duties as assigned.
Complies with all policies and standards.
Education / Experience
Requires a bachelor’s degree and one year of related experience, or equivalent experience acquired through the accomplishments of applicable knowledge, duties, scope, and skill reflective of the level of this position.
Candidate Requirements for Illinois Plan Only
RN licensed in Illinois.
Bachelor’s or Master’s degree in one of the following human services fields: Child, Family and Community Services, Early Child Development, Guidance and Counseling, Home Economics‑Child and Family Services, Human Development Counseling, Human Service Administration, Human Services, Master of Divinity, Pastoral Care, Pastoral Counseling, Psychiatric Nursing, Psychiatry, Psychology, Public Administration, Rehabilitation Counseling, Social Science, Social Services/Social Work, or Sociology.
LPN with one year experience in conducting comprehensive assessments and providing formal services for the elderly.
Pay Range: $22.94 – $38.79 per hour.
