American Traveler seeks an Occupational Therapist with at least 1 year of recent home health experience and proficiency in OASIS and HCHB for a home health assignment in the Chippewa Falls area.
Details
Home health setting serving geriatric patients
Territory up to a 60-mile radius from the hub office
Day shift, 8:00 AM to 5:00 PM, 5x8 hour schedule
Average caseload 5–7 patients per day
On‑call rotation required every 3rd to 4th weekend
EMR system is Homecare Homebase (HCHB); experience required
First‑time travelers are accepted
Locals are accepted and qualify for travel rate
Travel pairs accepted; each therapist assigned to their own territory
iPad provided; cell phone not provided
Mileage reimbursed at IRS standard rate
Requirements
Active OT license required for consideration
Minimum 1 year of recent relevant home health OT experience
Current BLS certification required
OASIS experience required
Proficiency in Homecare Homebase (HCHB) required
Valid driver's license and reliable personal transportation required
Must be flexible with assigned territory
Timely documentation and communication required
Additional Information
Provide occupational therapy services and complete OASIS documentation for home health geriatric patients
Adhere to timely documentation and communication standards
Separation of at least 6 months required for former permanent employees before returning as travelers, subject to HR approval
Dress code is scrubs of any color; jeans are not allowed
Completion of Hospice/Home Health Checklist required
Requested time off must be submitted at the time of application
