We are seeking a dedicated and patient‑focused Physical Therapist (PT) to join our rehabilitation team at our Skilled Nursing Facility. As a Physical Therapist, you will play a critical role in helping our residents improve mobility, reduce pain, and regain independence after illness, injury, or surgery. You’ll be part of a multidisciplinary team committed to providing the highest standard of care in a supportive and collaborative environment.
Responsibilities

Evaluate and assess patients to develop individualized treatment plans
Provide physical therapy services to patients recovering from injury, illness, or surgery
Collaborate with physicians, nursing staff, and other healthcare professionals to coordinate patient care
Monitor patient progress and adjust treatment plans as necessary
Educate patients, families, and caregivers on therapy techniques and home exercises
Maintain accurate and timely documentation in accordance with state, federal, and facility guidelines
Ensure compliance with all regulatory and facility standards
Contribute to team meetings and care planning for residents
Qualifications

Graduate of an accredited Physical Therapy program
Current and valid Physical Therapist license in the State of California
CPR certification (current and valid)
Experience in a skilled nursing or long‑term care setting preferred, but not required
Strong communication and documentation skills
Ability to motivate and encourage patients in a compassionate, respectful manner
