Richland, Ashland, Huron, Crawford, Marion, Morrow, Knox, Delaware & surrounding counties
Coverage Areas:
Multi‑county region across Central/North Central Ohio
Mileage Reimbursement:

Mon–Fri, 8:00am–4:30pm (Full Time)

Pay Rate:
$45-$47/hr
About The Role
We are seeking an experienced
Registered Nurse (RN)
to deliver high‑quality home health care across a broad multi‑county service area. This role requires a confident, autonomous clinician capable of managing a diverse caseload while providing exceptional patient‑centered care in the home setting. You will implement the full nursing process, ensuring safe, compliant, and compassionate care aligned with physician orders and organizational standards.
Key Responsibilities
Clinical Excellence: Provide skilled nursing interventions including wound care, medication administration, disease management, and other home‑based clinical procedures.
Patient Assessment: Conduct comprehensive physical and psychosocial assessments to develop, update, and evaluate individualized plans of care.
EMR Documentation: Complete accurate, timely clinical documentation for all patient interactions. Experience with Homecare Homebase (HCHB) is highly preferred.
Education: Teach patients and caregivers about disease processes, medication safety, home treatment techniques, and ongoing care needs.
Care Coordination: Collaborate with physicians, therapists, and interdisciplinary team members to ensure cohesive, holistic patient care.
Observations & Reporting: Monitor patient responses to treatment, document changes, and elevate concerns promptly to ensure safe and effective care delivery.
On‑Call/Weekend Requirements
1 on‑call week
1 weekend during the assignment
Required Qualifications
Experience: Home Health nursing experience required
Licensure: Active RN license valid in the state of Ohio
Certification: Current CPR certification
Attributes: Strong clinical judgment, ability to work independently, and comfort traveling across multiple counties
