Position Overview
The Home Health Registered Nurse provides skilled nursing care to patients in their homes across a multi‑county service area. This role focuses on delivering high‑quality home health services, coordinating care with physicians and interdisciplinary teams, and ensuring accurate documentation while meeting clinical and scheduling expectations.
Schedule & Work Details
Weekly Hour Guarantee: 36 hours
Schedule: Monday through Friday, 8:00 AM to 4:30 PM
On Call Requirement: One week and one weekend rotation
Coverage Area: Richland, Ashland, Huron, Crawford, Marion, Morrow, Knox, Delaware, and surrounding counties
Mileage Reimbursement: Reimbursed between patient visits only (not from first patient or last patient)
Pay Rate: $51-$53 per hour
Key Responsibilities
Provide skilled home health nursing services to patients in their homes in accordance with physician orders and established care plans
Perform patient assessments, monitor health status, and identify changes in condition requiring intervention or escalation
Deliver nursing interventions such as medication management, wound care, patient education, and disease management
Coordinate care with physicians, case managers, and interdisciplinary team members to support optimal patient outcomes
Accurately and timely document all patient care and visits in compliance with home health regulations and agency policies
Manage an assigned caseload while meeting quality, productivity, and scheduling expectations
Required Qualifications
Active Ohio Registered Nurse (RN) license
Prior home health nursing experience
