Position Overview

The Home Health Registered Nurse provides skilled nursing care to patients in their homes while supporting quality outcomes, productivity standards, and regulatory compliance. This role focuses on patient assessment, care coordination, education, and documentation while collaborating with an interdisciplinary care team to deliver safe, effective, and patient-centered home health services.
Schedule & Work Details

Monday through Friday, 8:30 AM to 4:00 PM
Weekly hour guarantee of 32 hours
Coverage area includes Olympia and surrounding areas
No on-call or weekend requirement
Mileage reimbursed between patient visits only, not from home to first patient or last patient to home
Pay rate: $53.00 per hour
Dress code allows any scrub color
Key Responsibilities

Provide skilled nursing care in patients’ homes according to physician orders and established home health standards
Conduct comprehensive assessments and ongoing evaluations to identify changes in patient condition
Deliver clinical services as outlined in the LHC Group job description, including medication administration, patient education, and care coordination
Collaborate with physicians, therapists, and interdisciplinary team members to develop and implement individualized care plans
Accurately document patient visits, assessments, and care provided in a timely manner
Manage productivity expectations while maintaining quality patient care and satisfaction
Ensure compliance with agency policies, professional nursing standards, and regulatory requirements
Required Qualifications

Current Registered Nurse licensure
Home health nursing experience required
Ability to work independently in the field while managing time and productivity expectations
Strong assessment, communication, and documentation skills
Working Conditions

Work performed primarily in patients’ homes within the assigned coverage area
Frequent travel between patient visits required
Exposure to conditions common in home health environments
