Supervisor, Population Health Care Management

Provides direct supervision and operational oversight of Population Health Case Managers and Lead Population Health Case Managers. Responsible for achieving program goals through coordination of care management activities, staff performance management, and workflow execution across population health initiatives. Supports the delivery of care coordination services that improve patient outcomes, reduce avoidable utilization, and address social drivers of health (SDOH).

Leads day-to-day operations of care management programs including transitions of care, chronic disease management, Interactive Home Monitoring (IHM), Remote Patient Monitoring (RPM), and Remote Therapeutic Monitoring (RTM). Ensures staff adherence to established workflows, documentation standards, and regulatory requirements. Partners with leadership and interdisciplinary teams to support quality improvement, health equity initiatives, and value-based care performance.

Responsibilities

Responsible for supervising Population Health Case Managers and Lead Case Managers; achieves operational and program goals through direct oversight of staff and care management activities

Supervises professional and support-level employees; sets goals and expectations, monitors performance, and provides coaching and development to ensure effective care coordination and patient engagement

Ensures policies, procedures, and workflows are consistently understood and followed across the team

Coordinates daily operations of population health programs, including care transitions, high-risk patient management, and digital health programs (RPM, RTM)

Ensures timely patient outreach, appropriate documentation, and effective execution of individualized care plans

Monitors team performance using established metrics (e.g., readmissions, emergency department utilization, care gap closure, program engagement); identifies trends and implements process improvements in collaboration with leadership

Supports implementation and optimization of workflows related to SDOH screening, documentation, and referrals; promotes accurate use of SDOH data, including Z-codes, to support care planning and quality reporting

Collaborates with internal and external stakeholders, including clinical teams, digital health partners, and community organizations, to ensure coordinated, patient-centered care

Develops solutions to routine operational challenges and escalates more complex issues to higher-level management as appropriate

May perform other duties as assigned

Minimum Requirements
Speech Language Pathology –
Education: Master’s Degree in Speech-Language Pathology. Experience: Must have acute care hospital experience. Prefer 3 years of demonstrated leadership experience. Licensure: Licensed to practice Speech-Language Pathology from the Virginia Board of Examiners for Audiology and Speech Pathology required. Eligible for Clinical Fellowship Year (CFY) or certified with Certificate of Clinical Competence (CCC) is required. American Heart Association (AHA) Health Care Provider BLS certification required.

Physical Therapy –
Education: Minimum-Bachelor’s Degree in Physical Therapy Prefer master’s or Doctoral Degree. Experience: Must have acute care hospital experience. Prefer 3 years of demonstrated leadership experience. Licensure: Valid license or registration to practice in the Commonwealth of Virginia in Therapy discipline required. American Heart Association (AHA) Health Care Provider BLS certification required.

Occupational Therapy –
Education: Minimum-Bachelor’s Degree in Occupational Therapy Prefer Master’s or Doctoral Degree. Experience: Must have acute care hospital experience. Prefer 3 years of demonstrated leadership experience. Licensure: Valid license or registration to practice in the Commonwealth of Virginia in Therapy discipline required. American Heart Association (AHA) Health Care Provider BLS certification required.

Physical Demands
Job requires standing for prolonged periods and frequently bending/stooping/traveling. Proficient communicative, auditory and visual skills; Attention to detail and ability to write legibly; Ability to lift/push/pull 20 – 50lbs. May be exposed to blood/body fluids and infectious disease.

Compensation
The starting base rate for this role is $76,420.00 annually. Individual compensation will be determined by the selected candidate's qualifications, previous work experience, and/or education.
