Role Description
This is a full-time, mobile position located in Mesa, Chandler, Gilbert, AZ. As a Nurse Practitioner at Golden Years Primary Care, LLC, you will provide high-quality, personalized care to patients, focusing on diagnosis, treatment, and preventative health measures. Responsibilities include conducting patient assessments, creating and implementing treatment plans, prescribing medication, and collaborating with healthcare team members to ensure comprehensive care. You will also work closely with patients to educate them about their health conditions and promote wellness.
Qualifications
Proficiency in conducting thorough patient assessments, diagnosing medical conditions, and developing individualized treatment plans
Skilled in prescribing medications and managing chronic and acute medical conditions
Excellent communication and patient education skills to promote health and wellness
Ability to collaborate effectively with a multidisciplinary healthcare team
Experience working with geriatric patients is highly preferred
Master's or Doctorate degree in Nursing, with a valid Nurse Practitioner license in Arizona
Certification in primary care, family practice, or gerontology is an advantage
Basic Life Support (BLS)/Advanced Cardiovascular Life Support (ACLS) certification
