- Other details: The facility is looking for a provider to cover on-site clinical hours and beeper call if needed. There are an average of 5 patients per shift. We have an urgent need for multiple CRNAs starting in November. Candidates must be willing to be credentialed at multiple locations, work 8-hour shifts Monday through Friday, and share weekday and weekend call as required. Providers must have an active AZ license, NBCRNA certification, at least one year of experience, and current ACLS and BLS certifications. They must be comfortable practicing independently, performing peripheral nerve blocks, and able to provide case logs of blocks performed, as training will not be provided. Candidates must also disclose any gaps in service, malpractice claims, or board sanctions. Providers should be willing to work nights and weekends as needed, and with call coverage responsibilities
