We are seeking a compassionate and highly organized

Licensed Social Worker

to join our interdisciplinary team. As a Field Social Worker, you are the primary advocate for our patients and their families. You will provide psychosocial assessments, emotional support, and resource coordination to ensure our patients live with dignity and peace during their end‑of‑life journey.
Key Responsibilities

Psychosocial Assessments:

Conduct initial and ongoing assessments of the patient’s and family’s emotional, social, and financial needs.
Counseling & Support:

Provide grief and bereavement counseling to patients and their loved ones, helping them navigate the emotional complexities of terminal illness.
Resource Coordination:

Assist families with long‑term care planning, funeral arrangements, and applications for community resources (Medicaid, VA benefits, meals‑on‑wheels).
Crisis Intervention:

Respond to urgent psychosocial needs or family conflicts that impact the patient’s care plan.
Interdisciplinary Collaboration:

Participate in weekly IDG meetings to coordinate care with nurses, chaplains, and physicians.
Documentation:

Complete clinical notes within 24 hours of the visit via EMR system.
Qualifications (Mandatory)

Education:

Master of Social Work (MSW) from a school accredited by the Council on Social Work Education.
Licensure:

Must hold a current, valid Social Work license in the State of Alabama (LMSW or LCSW preferred; LBSW considered based on experience and state regulations).
Experience:

Minimum of 1 year of experience in a healthcare setting (hospice, home health).
Transport:

Reliable vehicle, valid driver’s license, and active auto insurance (frequent travel within the service area is required).
Deep understanding of end‑of‑life care and the hospice philosophy.
Excellent verbal and written communication skills.
Ability to work independently in a mobile/field environment.
Proficiency in electronic medical record (EMR) documentation.
