JOB REQUIREMENTS
Licenses: Active Illinois medical license (CSR required)
Board: Board Certified in Physical Medicine and Rehabilitation
DEA: Active DEA Certification
Certifications: ACLS, BLS
Experience: Minimum 1 year of relevant experience
Other Qualifications: Proficiency with EMR (EPIC)
DUTIES & RESPONSIBILITIES
Provide inpatient rehabilitation care for adults with physical and neurological conditions.
Collaborate with an interdisciplinary team of therapists, nurses, and specialists.
Develop and oversee individualized rehabilitation plans.
Coordinate patient care transitions and ensure continuity of treatment.
Participate in documentation and communication via EPIC EMR.
Maintain compliance with hospital policies and credentialing standards.
Deliver high-quality, compassionate patient care aligned with OSF HealthCare standards.
