Job Description
We are seeking a Medical Case Manager/Behavioral Health Therapist to provide multi-disciplinary medical case management to HIV+ clientele with co-morbid conditions (i.e., substance abuse dependence and PTSD) with the primary goal of improving the client’s overall health and well-being. The Medical Case Manager will formulate and systematically review individualized client care plans, maintain progress logs, facilitate needed client referrals, and conduct comprehensive discharge planning. The Medical Case Manager will also facilitate psychoeducational client group sessions covering topics such as substance use, drug interaction, individual coping mechanisms, anger management, interpersonal relationship skill development, and generating healthy lifestyles.

Key Responsibilities
Conduct detailed intake evaluations and biopsychosocial assessments
Create, monitor, and revise individualized client care and treatment plans
Coordinate medical appointments and make referrals to mental health, housing, or social services
Conduct home visits and accompany clients to medical appointments when needed
Facilitate successful discharge planning and follow-up care
Behavioral & Psychoeducational Counseling
Provide individual counseling on topics including mental health, substance use, risk reduction, and behavior change
Facilitate group sessions on:
Drug interactions
Communication & relationship building
Building healthy lifestyles
Lead crisis intervention efforts including suicide prevention and behavioral de-escalation
Documentation & Compliance
Maintain accurate progress logs and case notes
Ensure timely, compliant documentation in line with program, ethical, and legal standards
Participate in outreach activities, organizational events, and community-based programming
Qualifications and Skills
Master’s Degree in Social Work, Psychology, Counseling, or related field (required)
Minimum of 3 years’ experience in social services or related care work
Licensure or Certification preferred: CADC, LPC, LCPC, LSW, LCSW, MISA
Strong knowledge of HIV care, behavioral health, and substance use disorders preferred
Cultural sensitivity to Black, Latino, and LGBTQ+ populations is essential
HIV/AIDS testing certification (or ability to obtain within 6 months)
Strong organizational, documentation, and time-management skills
