Case Manager I-Substance Abuse

Responsibilities
Provide comprehensive case management, support, and advocacy to individuals with low to moderate risk
eeds scores for criminogenic behavior/cognition.
Promote the health and wellbeing of individuals and families affected by maladaptive behaviors and cognition, substance abuse and mental illness.
Administer comprehensive, evidence-based screening and assessment to identify behavioral health needs and other risk factors requiring intervention.
Develop individualized plans of care that address medical and psychological needs, housing, vocational/educational needs, transportation, substance abuse and social support needs.
Provide case plan monitoring and ongoing support through the recovery process, coordinate discharge planning and aftercare plans, and linkage with community resources.
Facilitate referrals for further clinical assessment as indicated.
Document patient progress in the clinical record according to established standards.
Implement appropriate evidence-based behavioral health interventions for lower criminogenic risk populations.
Maintain a working knowledge of available behavioral health and social service providers in the community.
Participate in quality improvement initiatives and educational/research activities.
Perform other duties as assigned.
Additional Duties
Screening
Complete program appropriate screening per policy and procedure.
Participant orientation.
Complete appropriate consent to release and HIPAA forms.
Monitoring
Monitor overall compliance with case management plan.
Regularly review and update case management plan and referrals.
Maintain accurate and appropriate documentation in the appropriate database and/or case file.
Adherence to discharge procedure per programmatic policy and procedure.
Advocacy
Participant: Court – prepare timely information, verbal and written; consultation with court personnel, attorneys, and other court-related parties.
Program: Provide program information to outside entities; represent and promote program concepts to outside entities.
Training and Development
Attend training and development as required.
Provide training and documentation as required.
Maintain appropriate individualized training on current trends, in regards to the population served.
Drug Education
Plan and execute drug education for participants and families as required.
Document each educational session.
Qualifications
Education
Bachelor's degree in Social Work, Community Health and Human Services or a related field is required. Work experience may NOT substitute for education requirement.
Salary
Hourly Range: $19.80 - $32.15
Miscellaneous
MVR needed.
