General Responsibilities

To provide comprehensive case management, support, and advocacy to individuals possessing low to moderate risk
eeds scores for criminogenic behavior/cognition.

To promote the health and wellbeing of individuals and families affected by maladaptive behaviors and cognition, substance abuse, and mental illness.

To administer comprehensive, evidence‑based screening and assessment to identify behavioral health needs and other risk factors that may require further intervention.

To develop individualized plans of care that address medical and psychological needs, housing, vocational/educational needs, transportation, substance abuse, and social support needs.

To provide case‑plan monitoring and on‑going support through the recovery process and coordinate discharge planning through the development of aftercare discharge plans and linkage with community resources and other services to meet on‑going needs.

Key Roles & Responsibilities

Conduct evidence‑based screening and assessment to identify patient behavioral health and other social service needs and barriers to disease management.

Develop individualized plans of care for lower risk populations to address lower risk criminogenic, medical, psychological needs, housing, vocational/educational needs, transportation, substance abuse, and social support needs.

Provide case‑plan monitoring and on‑going support through the recovery process.

Facilitate referrals for further clinical assessment as indicated.

Coordinate discharge planning through the development of aftercare discharge plans and linkage with community resources and other services to meet on‑going needs.

Document patient progress in the clinical record according to established standards.

Implement appropriate evidence‑based behavioral health interventions for lower criminogenic risk populations, to address needs identified through initial screening and assessment.

Maintain a working knowledge of available behavioral health and social service providers in the community.

Provide and document relevant patient progress according to policy.

Update the assessment and intervention strategies in accordance with accepted national standards.

Participate in quality improvement initiatives and educational/research activities.

Perform other duties as assigned.

Additional Duties
Screening

A. Complete program appropriate screening per policy and procedure.

B. Determine program eligibility.

C. Participant orientation.

D. Complete appropriate consent to release and HIPAA forms.

E. Develop case‑management plans for participants.

Monitoring

A. Monitor overall compliance with case‑management plan.

B. Regularly review and update plan and referrals.

C. Maintain accurate and appropriate documentation in the appropriate database and/or case file.

D. Adhere to discharge procedure per programmatic policy and procedure.

Advocacy

A. Participant: Court a. Prepare timely information, verbal and written; b. Consult with court personnel, attorneys, and other court‑related parties.

B. Program: 1. Provide program information to outside entities; 2. Represent and promote program concepts to outside entities.

Training and Development

A. Attend training and development, as required.

B. Provide training and documentation as required.

C. Maintain appropriate individualized training on current trends, in regards to the population served.

Drug Education
Plan and execute drug education for participants and families as required; document each educational session.

Hourly Range:

$19.80 - $32.15

Qualifications
Education

Bachelor's degree in Social Work, Community Health and Human Services or a related field required.

Work experience may NOT substitute for education requirement.
