Aftercare & Outreach Manager
for the residential Programs (RESET, ES, TLP and GTP). The role is responsible for assisting adolescents and young adults with linkage and access resources and coordinating community outreach to increase awareness of available services.
Position: full‑time, 40 hours per week, evenings and weekends as needed for agency and youth needs. The agency offers a comprehensive benefits package including health coverage, 401k, and other ancillary benefits.
Responsibilities
Coordinate aftercare support and services for youth transitioning from the RESET, Emergency Shelter, Girls Therapeutic, and/or Traditional Living Programs.
Identify available and relevant resources for survivors of commercial sexual exploitation and youth aging out of foster care.
Develop discharge and aftercare plans with youth transitioning from agency programs.
Conduct post‑discharge follow up with clients.
Community outreach.
Required Education and Experience
Bachelor’s Degree or higher in Social Work, Psychology, or Human Services.
One to two years of experience working with children/youth with behavioral and emotional issues.
Knowledge of child growth and development.
Previous experience working with survivors of commercial sexual exploitation, preferred.
