Responsibilities:
Provide direct patient care to residents, including administering medications, performing vital signs, and assisting with activities of daily living
Supervise and mentor LPN/LVN staff to ensure high-quality care is delivered
Coordinate and lead shift report to ensure effective communication and continuity of care
Develop and implement care plans in collaboration with the interdisciplinary team
Monitor resident outcomes and identify opportunities for improvement
Collaborate with physicians, therapists, and other healthcare professionals to ensure comprehensive care
Maintain accurate and timely records of patient care and treatment
Participate in quality improvement initiatives and contribute to a culture of excellence
Requirements:
Licensed Practical Nurse (LPN) in the state of Georgia (compact license acceptable)
Experience in a long-term care setting (acute care experience a plus)
Ability to work varying shifts, including days, evenings, and weekends
Strong communication and leadership skills
Ability to work effectively in a fast-paced environment
