Nassau Rehabilitation and Nursing Center is seeking a dedicated and compassionate Physical Therapist to join our interdisciplinary rehabilitation team. This full‑time role offers the opportunity to make a meaningful impact on patient recovery and quality of life in a supportive skilled nursing environment.
Key Responsibilities
Consult with patients to evaluate physical conditions, symptoms, and medical history
Diagnose movement dysfunction through observation and assessment
Develop and implement individualized treatment plans
Coach patients in therapeutic exercises and stretches
Provide hands‑on therapy (e.g., massage) to relieve pain and promote healing
Monitor progress, adjust treatment plans, and maintain accurate patient records
Educate patients and families on in‑home care and recovery expectations
Candidate Requirements
Doctor of Physical Therapy (DPT) from an accredited program
Active state licensure as a Physical Therapist
Minimum of 2 years of experience preferred
Nursing home or skilled nursing experience preferred
Critical Qualifications
Proficiency in physical therapy techniques and modalities
Strong assessment and diagnostic skills
Compassion, patience, and excellent communication abilities
Familiarity with SigmaCare and Net Health preferred
