
Patient Care Technician - PCT

PURPOSE AND SCOPE: Functions as part of the dialysis health care team in providing safe and effective dialysis therapy for patients under the direct supervision of a licensed nurse, following organization policies and regulations set by the corporation, state and federal agencies.
PRINCIPAL DUTIES AND RESPONSIBILITIES
Patient Related
Provide patient‑specific education regarding adequacy measures and disease process.
Welcome patients and inquire about wellbeing since last treatment.
Evaluate vascular access for patency and perform cannulation and heparin administration as delegated.
Obtain pre‑ and post‑treatment vital signs, weight and cardiovascular parameters.
Initiate dialysis treatment per prescribed orders, monitoring blood flow and dialysate flow.
Identify intradialytic problems and intervene per physician orders.
Monitor patients’ response to dialysis therapy and document vital signs.
Discontinue treatment per protocol, evaluate patient, and apply hemostasis with appropriate dressings.
Record pre‑ and post‑treatment evaluations, weight, and vital signs.
Program dialysis machine for ultrafiltration goal, treatment time and UF modeling.
Ensure safety alarms are enabled and record accurate parameters on the Hemodialysis Treatment Sheet.
Document all information related to patient treatment and report significant changes to the registered nurse.
Perform machine safety checks and document any interventions for unusual patient status.
Obtain laboratory testing specimens and handle blood spills per facility policy.
Prepare supplies and equipment efficiently to prevent wastage.
Assist patients with self‑care under RN supervision.
Staff Related
Assist in training direct patient care staff on appropriate programs under nursing supervision.
Education/Communication
Follow all organization business policies, procedures and ethical practices.
Maintain environmental integrity and cleanliness.
Clean and disinfect dialysis machine surface, chair, equipment and surrounding area between treatments.
Prepare for dialysis treatment and perform pre‑treatment alarm testing.
Initiate Solution Delivery System (SDS).
Record Keeping
Complete and document Continuous Quality Improvement activities.
Enter all treatment data into the designated clinical application accurately and timely.
Review treatment sheets for completeness and ensure nursing signatures are documented.
Prepare initial patient chart for admission and compile paperwork in the chart.
Prepare/print lab requisitions for prescribed specimens.
Collect, label, prepare and store lab samples per laboratory specifications.
Ensure collection of lab specimens by appropriate courier.
Infection Control
Assist in collecting information for infection control audits.
Support staff and patient adherence to infection control practices.
Follow infection control policies and procedures.
Participate and reinforce infection control education of patients and families.
Transitional Care Unit
Follow newly admitted patients through their first four weeks of dialysis, coordinating transitional care.
Reinforce education and care‑related matters for new patients.
Coordinate with facility staff to schedule disciplines accordingly.
Assist patients in understanding home dialysis options and benefits.
Set and perform treatments on machines used in the transitional care unit.
Explain treatment options, including peritoneal dialysis set‑up.
Coordinate transition to patient’s modality choice.
Assist with supply ordering and inventory for the transitional care unit.
Physical Demands and Working Conditions
Require desk and computer work, patient interaction and travel between facilities.
Operate in a healthcare environment with controlled temperature and moderate noise.
Provide direct patient care involving heavy lifting and ambulation assistance.
Stand for prolonged periods, bend over, push or pull equipment up to 15 pounds, and lift solutions up to 40 pounds.
Program for reasonable accommodations for individuals with disabilities.
Employees required to pass Ishihara Color Blindness test; accommodations considered.
Education and Licenses
High School diploma or G.E.D. required.
Meet CMS‑approved state and/or national certification requirements within state or CMS timelines.
Maintain all appropriate state licensure and training.
Demonstrate commitment to organization culture, values and customer service standards.
Experience and Required Skills
Previous patient care experience in a hospital or related setting preferred.
Successful completion of the organization dialysis training program and CPR certification required.
