Job Summary
You will provide direct case management services to an adult mental health population. You will spend 50% of your time face‑to‑face with consumers in their homes and the community and have an opportunity to use various evidence‑based modalities (such as CBT, MI, etc.) to aid in the mental health recovery journey of your clients.
What You Will Do
Facilitate and coordinate psycho‑educational services for consumers and their family members on a variety of topics relating to wellness, health promotion, prevention, skill‑building, and recovery topics.
Offer on‑site and outreach services (including transport of consumers)
Fulfill case management services, including coordinating or directly facilitating linkages to services, service coordination, and treatment enrollment assistance.
Ensure consumers have community support in place to address identified needs.
Attend routine supervision and team meetings to review cases and discuss program operations.
Implement evidence‑based interventions (CBT, MI, etc.) and assessments to help individuals achieve their treatment goals, link to community resources, and provide inter‑agency coordination of treatment.
Participate in supportive counseling and crisis intervention.
Participate in the assessment and evaluation of consumers at intake/admissions, during ongoing recovery/service delivery, and at discharge.
Conduct case management intake assessments and formulate plans of care.
Perform reviews of progress and evaluate the effectiveness of goals.
Work with administrative staff to ensure that all services are documented based on billing and regulatory standards.
What We Need to See
Master's Degree in a Mental Health‑related field OR
Bachelor's Degree in a Mental Health‑related field, plus one (1) year post‑BA experience in the Mental Health field
Valid driver's license insurable by agency's automobile insurance
