About New Horizons
New Horizons is the largest mental health and addiction recovery provider on the Treasure Coast (and beyond), serving 15,000 children and adults annually through inpatient crisis services, a 24-hour help line and mobile response team, and outpatient programs conveniently located across Indian River, Martin, Okeechobee and St. Lucie counties. In addition, New Horizons assists 22,000 students in area schools, and we work closely with the courts, law enforcement, jails, and hospitals to help improve the health of individuals and the quality of life in our community.
Outpatient Care Coordinator Responsibilities
Welcome and assist clients in a professional and friendly manner
When appropriately credentialed, may perform vital signs as directed
Assist clients with telehealth appointments
Ensures all medication-related calls are returned within one business day
Ensures clients receive medications by managing refill requests, resolving prescription issues, and coordinating with providers and pharmacies, without making clinical decisions
Coordinates client services in collaboration with outpatient psychiatric providers and staff
Communicates with clients regarding appointments, medications, and follow-up needs
Works with scheduling staff to arrange appointments
Enter and maintain accurate information in the EHR
Complete required forms and documentation
Uses appropriate forms, both paper and electronic
Complete FASAMS admissions and quarterly updates
Ensures documentation is accurate, complete, and timely
Perform other duties as assigned
Requirements
High school diploma or equivalent required
1–3 years of experience in a healthcare, behavioral health, or outpatient setting
Proficiency in Microsoft Office (Outlook, Word, Excel)
Preferred Qualifications
Experience with scheduling, referrals, insurance authorizations, or care coordination preferred
Bilingual skills are a plus
Associate’s or Bachelor’s degree preferred
