Outpatient Care Coordinator
This role supports our psychiatric outpatient team with a primary focus on medication coordination. The coordinator manages refill requests, resolves prescription issues, ensures timely follow‑up with providers and pharmacies, and assists in scheduling, patient communication, and overall clinic flow while maintaining accurate documentation. Responsibilities

Welcome and assist clients in a professional and friendly manner.

When appropriately credentialed, perform vital signs as directed.

Assist clients with telehealth appointments.

Ensure all medication‑related calls are returned within one business day.

Manage refill requests, resolve prescription issues, and coordinate with providers and pharmacies without making clinical decisions.

Coordinate client services in collaboration with outpatient psychiatric providers and staff.

Communicate with clients regarding appointments, medications, and follow‑up needs.

Work with scheduling staff to arrange appointments.

Enter and maintain accurate information in the EHR.

Complete required forms and documentation.

Use appropriate forms, both paper and electronic.

Complete FASAMS admissions and quarterly updates.

Ensure documentation is accurate, complete, and timely.

Perform other duties as assigned.

Requirements

High school diploma or equivalent required; Associate’s or Bachelor’s degree preferred.

1–3 years of experience in a healthcare, behavioral health, or outpatient setting.

Proficiency in Microsoft Office (Outlook, Word, Excel).

Preferred Qualifications

Experience with scheduling, referrals, insurance authorizations, or care coordination.

Bilingual skills.
