Overview
The APRN is a clinical role that will provide exceptional medical care to older adult patients by conducting physical exams, ordering and interpreting diagnostic tests, and developing patient treatment plans under the supervision of a physician. This safety‑sensitive position involves direct patient care and medication administration. The incumbent reports to the Regional Manager (or similar) and works closely with other healthcare professionals to provide quality care.
Responsibilities
Practice medicine under the supervision of a physician using sound medical judgment and adhering to ethical considerations.
Provide care for patients with acute and chronic illness encountered in older adults.
Accountable for patient care and outcomes and appropriately seek consultation from specialists when needed.
Engage with hospitalists for patients in the hospital.
Meet all clinical obligations: accrediting agency compliance, CLIA, drug prescription compliance, HIPAA, state and federal regulations, safety regulations, etc.
Assess, diagnose, treat, manage, educate, promote health, and provide care coordination for patients with acute and complex chronic health needs using evidence‑based medicine.
Follow all internal policies and procedures.
Ensure completion of all operational tasks according to departmental guidelines.
Maintain complete and detailed records electronically in the EMR within 48 hours of each visit.
Provide patients with information needed to promote health, reduce risk factors, or prevent/manage disease or disability.
Analyze and interpret patient histories, symptoms, physical findings, and diagnostic information to develop appropriate diagnosis.
Interpret data and diagnostic results (blood gases, radiology, EKG, etc.) in collaboration with supervising physician.
Exhibit compassion, quality, loyalty, and conscientiousness in patient care.
Establish and maintain effective working relationships with patients, families, and team members.
Document continuing education per licensure and certification requirements.
Ensure patients are seen by the physician at every alternate visit.
Maintain patient confidentiality per policy and HIPAA requirements.
Enact proper chain of command for patient complaints.
Perform additional duties as assigned.
Position Requirements / Skills
Graduate of an accredited nursing school.
Certified nurse practitioner with current state licensure.
At least 1 year of professional nursing experience, including 1‑3 years as a primary‑care nurse practitioner.
Prior experience working with an electronic medical records system.
Proficiency in patient assessment and management nursing disciplines.
ACLS/BLS certification (as required by state).
Ability to manage emergency situations calmly and effectively.
Ability to read, analyze, and interpret professional journals and regulations.
Excellent written and verbal communication, teaching ability, and telephone skills.
Working knowledge of Google Suite and Microsoft Office for real‑time collaboration.
Physical Requirements
Full range of bodily motion, including handling and transferring patients, manual and finger dexterity, eye‑hand coordination.
Requires standing, walking, pushing, bending, kneeling, reaching at arm’s length and overhead for prolonged periods in a clinic.
Must be able to lift and move patients and medical equipment as needed (up to 15 lb).
Requires corrected vision and hearing to normal range.
Ability to sit for extended periods.
Ability to operate a motor vehicle and have own means of transportation.
