Community of Hope is seeking a Lead Integrated Emotional Wellness Clinician to join our cohesive medical team to provide brief assessments, screenings, crisis intervention and short‑term counseling services which will reduce barriers to mental health care and educate patients and staff about mental health and treatment options. This position is located at our Family Health & Birth Center in NE, Washington, DC.

What You’ll Do
Lead the initiatives to increase capacity and expertise surrounding perinatal mental health needs and services, including program design and implementation, and data evaluation.
Support and closely coordinate mental health care with the patient’s primary care team and, when appropriate, coordinate with other treating mental health providers. Screen and assess patients for common mental health and substance use disorders. Consult with supervisor on difficult to engage clients.
Educate patients about common mental health and substance use issues and available treatment options. Help identify and address related problems affecting the health of client, such as housing, vocational rehabilitation, and social integration, and coordinate service delivery with other health center staff and community providers.
Provide crisis intervention per the clinician’s licensing and COH guidelines, to include addressing suicidal/homicidal ideations.
Provide short‑term, problem‑focused therapy for no more than 5 patients, unless approved by the supervisor and for up to 8 sessions per patient as deemed appropriate. Short‑term therapy will focus on goal setting, change, and utilizing brief interventions.
Maintain a daily schedule of at least 6 patients seen per day. Complete a treatment plan for all short‑term therapy clients by the second scheduled session. Monitor client progress through the course of treatment, reevaluating and adapting the treatment plan at least monthly from initial treatment plan date.
Collaborate with PCPs and psychiatric nurse practitioners to support patients who are not improving as expected. This collaboration may include treatment plan changes including new medications or psychosocial treatments, or appropriate referrals for additional services.
Facilitate and appropriately document referrals for clinically indicated services outside the primary care clinic (e.g., domestic violence safe shelter, mental health specialty care, core service agency, and substance use treatment).
Provide leadership of emotional wellness integration with the midwifery and maternal and child health teams, ensuring that activities align with COH policies, procedures, and strategic goals.
Meet with supervisees regularly and provide real‑time feedback, training, support, and coaching as needed. Create meeting agendas, send meeting agendas in advance when possible, and document concerns promptly as needed.
Perform other tasks as requested by supervisor.
Requirements
Must‑Haves
LICSW, LPC, or equivalent licensure required. (Any provider licensed in Maryland who provides a copy of their license and MD CDS to Talent Management is also eligible for MD licensure reimbursement.)
Minimum of two years of work experience in a clinical environment.
At least two years of supervisory experience.
Strong interpersonal and organizational skills.
Demonstrated ability to collaborate effectively in a team setting.
Experience in screening for common mental health symptoms.
Experience with assessment and treatment planning for common mental health and/or substance use disorders.
Familiarity with brief, structured intervention techniques.
Demonstrated ability to multi‑task and problem solve.
Ability to work with technology and electronic health records.
Ability to travel between Community of Hope sites.
Ability to work flexible hours, including evenings and weekends.
Proof of required vaccinations is required. This includes, but may not be limited to, Flu and Covid. COH will consider requests for reasonable accommodations for anyone who cannot be vaccinated for a religious or medical reason, subject to applicable law.
